Nutritional Screening at Nursing homes or Residential Aged Care dietitian Facilities
Nutrition/Hydration risk screening with simple and rapid validated screening tools can be performed with nursing home or aged care residents, to detect clients who are nutritionally at risk.
We use the Nestle Mini Nutritional Assessment-short form (MNA-SF) to screen for Nutritional status, and The Frail Scale Risk Assessment and/or the SARC-F to screen for frailty and sarcopenia.
A Comprehensive detailed nutritional assessment will be performed thereafter in those clients identified as at risk of malnutrition or who are malnourished. A nutritional care plan and education will be provided and can be implemented to improve the patient’s nutritional and overall condition.
“Early identification of nutrition and hydration issues can inform tailored care and prevent problems such as pressure injuries, incontinence, falls and delirium.”
