Dietitian 2UR Door provides a variety of Fee payments & rebates; including: Home care packages, National Disability Insurance Scheme (NDIS), bulk billing for Department of Veteran’s Affairs (DVA) clients, Medicare Rebates for Chronic Disease Management (CDM) Plans, Private Health insurance claims through Private Health funds, Traffic Accident Commission (TAC) and Work cover.

Home Care Package (HCP) Clients:

More seniors are choosing to stay independent at home using their HCP. We provide dietetic services to clients with these packages (including nursing homes and Residential Aged care Facilities) to assist them remaining healthy and happy:

National Disability Insurance Scheme (NDIS) Clients:

We are able to work with self-managed OR Plan managed NDIS clients via a service agreement to assist them to improve their overall health, nutrition & wellbeing. We have valid NDIS Worker Screening Checks.

Department of Veteran’s Affairs (DVA):

Requires a GP referral (D904 Referral form).

Gold card holders will be bulk billed for all conditions.

White card holders will be bulk billed for specific conditions, such as service-related conditions. Please visit the DVA website for more information. www.dva.gov.au

Chronic Disease Management (CDM) Plan:

Chronic conditions such as Diabetes, cancer, high cholesterol, weight concerns and IBS. Requires a GP referral-A GP Management Plan (GPMP) – (item 721) and Team Care Arrangements (TCAs) – (item 723). Dietitian services can be claimed under Item number 10954. Medicare rebates of $56.00 will be reimbursed for each consultation (up to a maximum of 5 visits per year under a CDM Plan). Full consultation payment due on the day and rebate subsequently claimed via medicare.

Private Health Insurance Claims:

Most private health funds offer a rebate for Dietitian fee’s and no GP referral is required. The amount of the rebate provided varies according to individual insurance and cover levels, and can be claimed after full payment. Please contact individual private health funds and quote Item number 800 Home/Out of rooms consultation for the rebate amount.
